HEALTH ASSESSMENT Immediately, we will conduct an assessment by phone and if necessary in person. The goal of this needs assessment is to get a better understanding of each person's unique needs. We assess the following: Medical condition Physical needs Emotional needs Preferred social environment Location preference Budget for care and housing Based upon these needs, we will then determine which homes will best fit your loved ones needs and provide tours of several State Licensed Assisted Living Facilities and/or homes until we find one you are happy with. We will negotiate the move in date, care requirements, and pricing with the home of your choice. FACILITY SCREENINGS How do you go about determining which facility is the best for your loved one?
