Answer: The Kansas City Metropolitan Physician Association (KCMPA) is a physician-led independent practice association working to make health care work better for everyone. KCMPA's collaborative network enables seamless, coordinated care across the care continuum. The capabilities KCMPA has built empower quality measurement and improvement and provide understanding of the total cost of patient care. The adult primary care subset of KCMPA was accepted as an Accountable Care Organization (ACO) into the Medicare Shared Savings Program with responsibility for 12,000 Medicare beneficiaries on January 2, 2013. KCMPA represents independent physicians on the board of the regional Health Information Exchange called the Lewis And Clark Information Exchange (LACIE) and is authorized to offer significant discounts to practices that want to connect to LACIE. KCMPA stands up a Data Network that connects more than 25 EMR systems among the 40+ member practices. The Data Network allows robust exchange of clinical data to improve care and reduce duplication. The principles of the Patient Centered Medical Home are the foundation of KCMPA. Most primary care practices have achieved the NCQA's Level 3 PCMH designation. Care compacts are developed with specialists to build out the Patient Centered Medical Neighborhood. KCMPA adopts evidence-based guidelines to reduce variations in care and optimize resources. Care coordination is a cornerstone of the KCMPA. ACO practices all have embedded care coordinators who are trained as nurses or social workers and focus on high risk, medically and socially complex patients. All KCMPA interventions come from the physician perspective seeking to harness the will of independent physicians to transform the health care system in Kansas City toward the triple aim of improved patient experience, improved health of populations and lower per capital cost.
