In partnership with the following hospitals and cardiac social workers at: New York-Presbyterian/Columbia University Medical Center, New York-Presbyterian/Weill Cornell, New York-Presbyterian-Morgan Stanley Children's Hospital, New York Hospital | NYU Langone Health, Northwell Health, Montefiore Medical Center, Mount Sinai Hospital, Westchester Medical Center, and Stony Brook University Hospital. Our programs support heart surgery patients and their families in four unique ways: Emergency Fund, Caregiver Food Delivery Program, Hope 4 Heart Days, and Community Events.
