Answer: Community Health Services (CHS) is a network of Federally Qualified Health Centers that provide medical, dental and behavioral health care in six counties in northwest Ohio including: Henry, Hancock, Allen, Huron, Wood and Sandusky. CHS operates nine medical offices in the region and our main campus is based in Fremont, Ohio. CHS providers offer a broad spectrum of care in specialties such as Family Practice, Pediatrics, Internal Medicine, Obstetrics, Gynecology, Behavioral Health and Dentistry. CHS is accredited by the Joint Commission and is certified as a Primary Care Medical Home or PCMH.
